Botox is a quick, no-downtime treatment — but what you do in the first 24 hours helps your results settle evenly and lowers your risk of bruising or the product migrating. This guide covers exactly what to do, what to avoid, how long recovery takes, the side effects that are normal, and when you can get back to the gym.
Botox aftercare: the first 24 hours
For the first day after your appointment:
Do:
- Stay upright for at least 4 hours. Avoid lying flat or bending over for long periods.
- Gently keep your face active for the first hour — natural expressions like smiling and raising your brows are fine.
- Apply a cool compress (lightly, no pressure) if you have minor swelling.
- Sleep on your back the first night if you can.
Avoid:
- Rubbing, massaging, or pressing the treated areas for 24 hours — this can move the product away from where it was placed.
- Strenuous exercise (more on this below).
- Heat — saunas, hot tubs, hot yoga, sunbeds, and very hot showers for 24 hours.
- Alcohol for 24 hours (it thins the blood and increases bruising).
- Blood-thinning products unless medically necessary — aspirin, ibuprofen, fish oil, and vitamin E can worsen bruising.
- Facials, microneedling, or other facial treatments on the area for about 2 weeks.
- Makeup on the injection sites for the first 24 hours, and don’t rub it in when you do reapply.
Can you work out after Botox?
Skip strenuous exercise for the first 24 hours. Light walking is fine the same day, but avoid anything that raises your heart rate or blood pressure significantly — running, weightlifting, HIIT, cycling, and hot yoga.
Two reasons:
- Increased blood flow can raise your risk of bruising and swelling at the injection sites.
- Bending, inversions, and lying flat (common in yoga and floor workouts) go against the “stay upright” rule that helps the product settle.

Botox has essentially no downtime — you can return to most normal, non-strenuous activities right away. Here’s the typical timeline:
